Description
As a Registered Nurse in Home Health, you will provide skilled, patient-centered care in the community. This position offers flexibility in structuring your day, manageable caseloads, and meaningful one-on-one patient care. You will perform assessments, develop care plans, and educate patients and families, all while working independently in a supportive team environment.

Requirements

EMR System: HCHB preferred
Comfort with documentation and EMR systems (HCHB preferred)
Ability to work independently and manage a structured caseload
Valid driver''s license, reliable transportation, and auto insurance
Clear, professional communication with patients and team members
Active Registered Nurse (RN) License in State of Colorado (or active NLC compact license)
Strong assessment and critical thinking skills
Home health experience preferred, but not required
