Requirements:

2 years of Home Health & Acute Care experience - Required
OASIS - Required
BLS - Required
Matrix EMR experience highly preferred
Above-average technology skills required, including use of laptop, iPad, and iPhone for documentation
Experience with CHF, Hypertension, Diabetes, Total Joint Replacement, and Oncology/Cancer patients
Travel pairs accepted
No locals within 50 miles

Schedule:

Monday-Friday, 8:00 AM-4:30 PM
