Description

American Traveler is hiring an experienced Home Health RN for a Days position requiring home health experience and an active IL RN license.

Details

• Home Health setting providing comprehensive skilled services to patients recovering at home
• Services include catheter care, chronic disease management, telemonitoring, diabetes management, home health aide services, infusion management, laboratory testing, medical social service, nutrition instruction, PT/OT/speech therapy, tube feeding, and wound/ostomy care
• Caseload of approximately 24-26 patients
• Territory within a 15-mile radius, which may increase if census is low
• Uses Matrix EMR and will be provided an iPad for documentation
• Performs some telehealth/virtual visits in addition to in-home visits
• Schedule is Monday-Friday, 8:00 AM-5:00 PM, 40 hours guaranteed
• No weekend requirements
• No on-call requirements
• 2-day orientation, with the first day in the office and second day in the field

Requirements

• Active IL RN license required
• Compact license not accepted
• Home health experience required
• Active BLS certification or willingness to obtain
• Valid driver's license and car insurance required
• Strong Med Surg experience with some home health experience will be considered
• OASIS and start of care experience strongly preferred
• COVID and flu vaccination required with no declinations approved
• Mileage reimbursed at the standard IRS rate

Additional Information

• Prescribe, coordinate, and deliver patient care through the nursing process while maintaining professional nursing standards
• Supervise CNAs and coordinate care with other healthcare workers
• Perform comprehensive initial and ongoing assessments covering physical, psychosocial, and environmental factors
• Initiate, re-evaluate, and modify the Plan of Care as needed, including discharge planning with patients, physicians, and caregivers
• Complete OASIS data and documentation in accordance with regulatory guidelines
• Assess educational needs of patients and families and implement appropriate teaching strategies
• Promote a clean and safe care environment using error prevention practices
• Maintain high standards of privacy and confidentiality
• Local candidates are preferred; candidates too far from the territory may not be considered
• No more than 5 days of RTO can be requested during the assignment
