Job Summary:
Home Healthcare RN in the Boston area. This role involves assessing patient conditions, implementing care plans, and providing ongoing education and support to patients and their families within a community health setting. The position requires performing admission processes and case management responsibilities in accordance with agency policies and standards, utilizing electronic documentation and interdisciplinary communication.

Job Details:

  • Schedule: Days shift, 8 hours per shift, minimum of 40 hours per week
  • Required Qualifications: Certifications and Licenses: Valid nursing license; Education: Registered Nurse (RN) degree; Experience: 1 year Certified Home Care experience, 1 year Case Management experience; Location: Boston area; Years of Experience: 1 year

  • Float: Float: Float rotation may be required.
  • Additional Information: Ability to travel to cases as assigned. Weekend rotations are scheduled once every 6 weeks. Start date is contingent upon credentialing clearance; if not cleared by the posted start date, a rescheduled date will be necessary.
