Job Details
- Schedule: Days shift, 8 hours per day, minimum of 40 hours per week.
- Required Qualifications: RN license in California, BLS certification, at least 2 years of clinical experience including patient assessment, care management, medication administration, wound care, IV therapy, and documentation using Epic charting system. Community hospital experience and travel experience are required.
- Preferred Qualifications: Experience with adult and geriatric patient populations, home infusion therapy, and managing complex wounds or medical equipment.
- Float: Float to areas that need help.
- Additional Information: The RN will be responsible for the assessment, coordination, delivery, and evaluation of in-home nursing care, including patient education and transitions of care. The workload involves approximately 5 visits per day, with interdisciplinary support including physical therapy and interpretation services. The position is open to nurses willing to work across Stanislaus County, San Joaquin County, and Merced County.
