Travel Physical Therapist (Home Health) – North of Stockton, CA

Connected Health Care, a leading healthcare recruitment and staffing firm, is partnering with a respected home health agency north of Stockton, California, to hire a Travel Physical Therapist for a 13-week contract. Compensation & Benefits

Responsibilities

• Evaluate patients in their homes and develop individualized treatment plans
• Provide skilled physical therapy services to improve mobility, strength, balance, and overall function
• Educate patients and caregivers on home exercise programs, fall prevention, and safe mobility techniques
• Monitor patient progress and modify treatment plans as needed
• Collaborate with physicians, nurses, occupational therapists, speech therapists, and other members of the interdisciplinary team
• Complete accurate and timely electronic documentation
• Maintain compliance with home health regulations and agency policies

Qualifications

• Doctor of Physical Therapy (DPT) or Bachelor's/Master's Degree in Physical Therapy from an accredited program
• Current California Physical Therapist License (or eligibility to obtain)
• Current BLS Certification
• Home Health experience preferred but not required
• Valid driver's license and reliable transportation
• Strong communication, organization, and time management skills
• Ability to work independently while managing a flexible schedule
