Job Summary
Physical Therapist, Home Health. The role provides in-home physical therapy services for geriatric patients, with an average daily caseload of 57 patients. Responsibilities include patient assessment, rehabilitation interventions, home exercise programs, and patient and family education.
Job Details
- Schedule: Day shift, 8-hour shifts, with a minimum of 40 hours per week.
- Required Qualifications: Minimum of 1 year of recent relevant experience; Basic Life Support (BLS) certification; proficiency in Homecare Homebase (HCHB); OASIS experience for clinicians completing Start of Care (SOC) home health visits; license in hand at submission; active drivers license; and personal transportation.
- Additional Information:
- On-call rotation may be required.
- Monday orientation is required.
- Timely communication and documentation are mandatory.
- Travelers must remain flexible regarding territory assignments.
- Charting must be completed by 9:00 a.m. the following morning.
- Scrubs of any color are permitted; jeans are not permitted.
- An iPad is provided for clinical documentation.
- Hospice/Home Health checklist completion is mandatory.
- Requested time off must be submitted at the time of application.
- Travel pairs may be accepted, with each clinician assigned to an individual territory and required to have personal transportation.
- Clinical experience may include acute care, outpatient clinics, rehabilitation hospitals, skilled nursing facilities, body mechanics, cervicalgia, low back pain, postoperative orthopedic and spinal procedures, spinal conditions, joint replacements, cognitive disorders, multiple sclerosis, muscular dystrophy, Parkinsons disease, post-stroke rehabilitation, spinal cord injuries, traumatic brain injuries, home exercise programs, and patient and family education.
