Job Summary

Physical Therapist Home Health provides skilled home health services in a one-to-one patient setting. The position includes day shifts, patient visits in the community, and documentation using designated home health systems. Weekend support may be required.

Job Details

• Schedule: Day shifts, 8 hours per shift, 40 hours per week; start date is 11/30/2026.
• Required Qualifications: At least 1 year of skilled Home Health experience; Basic Life Support (BLS) certification; active Oregon RN license before the start date; OASIS experience; Home Care Home Base (HCHB) experience; reliable personal transportation.
• Additional Information: Skilled Home Health assignment with a 1:1 patient ratio; productivity expectation of approximately 30 points per week and up to 6 patients per day. Timely communication and documentation are required. Weekend support may be required.
