Job Summary
Physical Therapist Home Health provides skilled home health physical therapy services in a one-to-one patient setting. The position involves managing up to six patients per day, meeting an approximate productivity expectation of 30 points per week, and completing timely clinical documentation. Weekend support may be required, while no on-call coverage is scheduled.
Job Details
- Schedule: Day shifts of 8 hours, totaling 40 hours per week. Four 10-hour shifts and three 12-hour shifts are not available. Assignment dates are 11/30/2026 through 03/01/2027.
- Required Qualifications: At least 1 year of skilled home health experience; Basic Life Support (BLS) certification; active Oregon registered nurse (RN) license before the start date; OASIS experience; proficiency with Home Care Home Base (HCHB); reliable personal transportation; active drivers license; timely communication and documentation skills.
- Additional Information: Skilled home health setting with a one-to-one patient ratio and up to six patient visits per day. Weekend support may be required, and no call requirement is scheduled. Business-casual attire is required, with scrubs permitted. Monday orientation is required. A home health checklist must be completed.
