Job Summary
Physical Therapist, Home Health Location not provided. This position provides home-based physical therapy services to adult and geriatric patients. The role includes patient and family education, timely clinical documentation, and communication with the interdisciplinary care team.
Job Details
- Schedule: Day shifts, 8 hours per shift, with a minimum of 40 hours per week.
- Required Qualifications: Minimum of 1 year of recent relevant experience; current Basic Life Support (BLS) certification; active license in hand at the time of submission; Outcome and Assessment Information Set (OASIS) experience for clinicians completing start-of-care visits; proficiency with Homecare Homebase; active drivers license and personal transportation.
- Preferred Qualifications: Charting system experience preferred.
- Additional Information:
- Patient population includes adults and geriatric patients.
- Typical patient ratio is up to 6 patients.
- Geriatric cases may include neurological, orthopedic, cognitive, and post-surgical conditions.
- Timely communication and documentation are required.
- Monday orientation is required.
- An on-call rotation may be required.
- A home health checklist must be completed.
- Scrubs of any color are required; jeans are not permitted.
