Job Description:

The Physical Therapist will primarily work in a home health setting, conducting regular visits and assessments including start of care, recerts, and resuming care. The typical workload involves 5-6 visits per day, excluding travel and add-on assessments or admissions. The therapist may occasionally be asked to perform safety evaluations or provide a one-time teaching visit in a hospice situation.

Responsibilities:

• Conduct start of care, recertifications, and resumption of care.
• Perform 5-6 regular visits per day when not handling admissions.
• Collaborate with clinical coordinators and other PTs for field support.

Requirements:

• BLS certification
• ND Physical Therapy License
• Current Driver's License & Auto Insurance
• Preferred experience with Home Health and OASIS training
