Travel Physical Therapist (PT) – Home Health / Skilled Nursing

Sarasota, Florida

We are seeking a Travel Physical Therapist (PT) for a 13-week contract in Sarasota, Florida, working with patients in a skilled nursing setting with a home-health-focused approach. This is a great opportunity for a PT who enjoys working with a geriatric population, promoting functional independence, and helping patients safely return to their prior level of function.

Experience: SNF or home health experience preferred

Responsibilities

Evaluate patients and develop individualized plans of care
Provide skilled physical therapy interventions focused on mobility and functional independence
Treat patients with a variety of medical, orthopedic, neurological, and age-related conditions
Provide gait, balance, transfer, strengthening, and mobility training
Assist patients with safe use of assistive devices
Monitor patient progress and adjust treatment plans as needed
Collaborate with nursing, therapy, and other members of the interdisciplinary team
Complete timely and accurate clinical documentation
Educate patients and caregivers on safety, mobility, and home exercise programs
