Job Summary
Physical Therapist, Home Health. This position provides home-based physical therapy services for adult and geriatric patients, including rehabilitation related to neurological, orthopedic, and spinal conditions. The role requires timely communication, thorough documentation, and independent management of daily patient visits.
Job Details
- Schedule: Day shifts consisting of 8-hour shifts, with a minimum of 40 hours per week.
- Required Qualifications:
- Minimum of 1 year of recent relevant experience
- Active Basic Life Support (BLS) certification
- License in hand at the time of submission
- Proficiency in Homecare Homebase (HCHB)
- OASIS experience required for clinicians completing Start of Care (SOC) home health visits
- Active drivers license and personal transportation
- Additional Information:
- Provides care for adult and geriatric patients, including cases involving Parkinsons disease, multiple sclerosis, muscular dystrophy, stroke rehabilitation, traumatic brain injury, spinal cord injury, cervical and lumbar conditions, and joint replacement recovery.
- Responsibilities include home exercise programs, patient and family education, body mechanics instruction, and postoperative rehabilitation.
- Daily workload may include 57 visits or a minimum of 30 points per week.
- Timely communication and documentation are required.
- On-call rotation may be required.
- Orientation is held on Monday.
- Scrubs are required; jeans are not permitted.
- An iPad is provided for clinical documentation.
