Job Description:

The Home Health Physical Therapist is responsible for providing therapy services to patients in a home health setting. Must have HCHB experience, no exceptions.

Responsibilities:

• Provide physical therapy treatments to patients as needed.
• Document patient progress using Meditech and RehabConnect.

Requirements:

• Current licensure to practice in NC.
• Experience in an inpatient rehab hospital preferred.
• Proficiency in safe patient handling techniques and familiarity with protocols to prevent complications.
• Good time management and critical thinking skills.
