Location: St. Albans, VT
Contract: 13 Weeks
☀️ Shift: Sunday–Thursday | 8:00 AM–4:00 PM
⏰ Hours: 37 Hours/Week
Specialty: Home Health & Hospice
EMR: McKesson
Call: On-Call Rotation Required
Mileage: Reimbursement Available
✅ Requirements
Active Vermont RN License required upon acceptance
Minimum 3 years of Home Health / Hospice RN experience
Experience with OASIS documentation
Experience serving as a case manager for home health or hospice patients
Experience with high-tech nursing skills, including IV therapy, wound VAC, and complex wound care
Strong patient assessment, care planning, and coordination skills
Ability to collaborate with physicians, LPNs, therapists, social workers, and other interdisciplinary team members
Strong documentation and patient/family education skills
Ability to provide safe, patient-centered care in home and hospice settings
Valid driver's license and ability to travel between patient visits
