Description

American Traveler is hiring an RN for a Home Health position requiring a UT or compact RN license and at least one year of home health, hospice, or acute care experience.

Details

  • Home Health setting serving adult and pediatric patients in their home environment
  • Four 10-hour day shifts per week, Monday, Wednesday, Friday, with every other Saturday and Sunday
  • Requires ability to commute to the office twice daily to electronically transfer patient files
  • Uses Epic for charting

Requirements

  • Active UT or compact RN license required
  • Current BLS certification (AHA) that does not expire before start date
  • ASN or Nursing diploma required
  • Minimum 1 year of experience in home health, hospice, or acute care setting preferred
  • Valid driver's license with reliable transportation and acceptable driving record
  • Ability to transfer and assist patients independently, including lifting, bending, and navigating stairs or uneven surfaces
  • Basic computer skills and strong verbal and written communication skills
  • DOB and last 4 of SSN required for consideration

Additional Information

  • Assess, plan, implement, and evaluate individualized care plans for home health and hospice patients
  • Coordinate and collaborate with interdisciplinary care teams, including physicians and support staff
  • Deliver care in compliance with hospice/home health policies, legal, and regulatory standards
  • Promote patient and family education and prepare patients for transitions in care
  • RTO must be submitted with candidate profile
  • Returning Intermountain employees must be separated from the facility for at least one year before being eligible for a travel assignment
