American Traveler is hiring an experienced Home Health RN with hospice experience and a current driver's license to serve patients within a 60-mile radius of Dickinson.

Details

  • Home health and hospice care setting serving adults, geriatric, and pediatric patients
  • Provides care in patients' homes, nursing homes, and other residential settings within a 60-mile service radius around Dickinson
  • Uses HomeCare HomeBase EMR system
  • Monday through Friday schedule, 8:00 AM to 5:00 PM
  • On-call rotation required for evenings and weekends
  • 24-hour on-call coverage required for weekends and holidays
  • Holiday coverage required
  • No clinical involvement in this role

Requirements

  • Active ND or Compact RN license required
  • BLS certification required
  • Minimum 1 year of nursing experience required
  • Home Health and Hospice experience required
  • Current driver's license and auto insurance required
  • Copy of driver's license required for consideration
  • Two professional references required: one supervisor from within the last 12 months worked and one peer or supervisor from within the last 3 years worked, including employment dates and rehire eligibility

Additional Information

  • Conduct initial client and family assessments to determine home health needs, including physical assessment and illness history
  • Regularly reevaluate client nursing needs and update nursing diagnoses based on assessment data
  • Develop and revise care plans based on client status, diagnosis, and family input, incorporating therapeutic, preventive, and rehabilitative actions
  • Administer medications and treatments as prescribed by the physician
  • Provide health care instructions to clients based on assessment and care plan
  • Identify and implement discharge planning needs prior to client discharge
  • May serve as Case Manager when assigned, coordinating care for an assigned caseload
  • Prepare clinical notes and update the primary physician at least every 60 days
  • Communicate with physicians regarding client needs, condition changes, and obtain orders as required
  • Coordinate necessary equipment and services for client care
  • Instruct, supervise, and evaluate home health aide care provided
  • Any solid color scrubs permitted
  • Must not have been directly employed (full time, part time, or PRN) by any CommonSpirit-affiliated facility, including CHI and/or Dignity, within the past year
  • RTO must be submitted upfront, as additional time off requests will not be approved after submission
