  • Position involves assessing patient physical, psychological, social, spiritual, educational, developmental, cultural, and discharge planning needs.
  • Reviews patient history, physical exams, diagnostics, and laboratory data, reporting abnormal results to physicians and caregivers.
  • Formulates a plan of care, including discharge planning, based on assessment data and input from patients, families, and the multidisciplinary team.
  • Implements the care plan through direct patient care, delegation, and supervision of healthcare team activities, complying with policies, procedures, standards of care, and regulatory requirements.
  • Provides care based on physician orders and the nursing plan of care, promoting continuity of care by clear communication.
  • Evaluates patient and family responses to care and education, revising the plan as needed to meet evolving needs.
  • Documents assessment, planning, implementation, and evaluation in the patient record, ensuring accuracy, timeliness, and adherence to policies.
  • Participates in research activities, education, role development, and mentorship within the clinical setting, promoting excellent patient outcomes through timely, effective, and safe care.
  • Interacts with all levels of staff, physicians, patients, and external contacts to support comprehensive patient care and foster collaboration.
  • Performs all functions according to established policies, procedures, regulatory and accreditation requirements, as well as professional standards.
