Description

American Traveler is hiring a Discharge Planner RN with Med/Surg case management experience for a daytime, Monday through Friday schedule.

Details

  • Performs case management and coordinates discharge planning for age-appropriate inpatients with chronic diseases and special health care needs
  • Supports Multi-Service Unit, Intensive Care Unit, OB Units, and Adolescent Care Unit patient populations
  • Maintains tracking system for patients referred for admission to other healthcare facilities
  • Performs chart reviews for requested services related to return to facility or discharge home
  • Monday through Friday schedule, 8:00 a.m. to 5:00 p.m. with a one-hour lunch (8-hour shifts)

Requirements

  • Active RN license (AZ or Compact) required
  • Current BLS certification (AHA) required and must be maintained
  • Minimum 2 years of experience in discharge planning required
  • RN experience in a clinical, medical/surgical, acute, and/or critical care setting required

Additional Information

  • Required to attend staff meetings when scheduled
  • Required to attend New Employee Orientation, scheduled every other Monday
  • Must be cleared 7 days prior to start date or start date will be pushed to next scheduled orientation
