Connected Health Care is seeking an experienced and motivated Home Health Physical Therapist Assistant (PTA) for a 13-week local travel assignment based in Independence, MO. This opportunity is with one of our trusted healthcare partners and is ideal for clinicians who excel in a home health setting and are comfortable working independently while supporting high-quality patient care.
The ideal candidate will have a strong home health background and be comfortable assisting with patient care plans, documentation, and traveling throughout the assigned Missouri service area.
Job Responsibilities
- Provide skilled physical therapy assistance to patients in their homes across Missouri service areas
- Implement treatment plans established by the supervising Physical Therapist
- Assist with patient mobility, therapeutic exercises, and functional training
- Monitor patient progress and report changes to the supervising PT
- Document patient visits accurately and in a timely manner
- Educate patients and caregivers on prescribed home exercise programs
- Maintain compliance with all home health regulations and standards
- Work independently while ensuring high-quality patient care
Education & Certification Requirements
- Associate degree from an accredited Physical Therapist Assistant program
- Active Physical Therapist Assistant (PTA) license in Missouri required
- Current CPR/BLS certification
- Home health experience preferred
- Experience with home health documentation systems preferred
