Key Responsibilities
- Provide occupational therapy treatment services to residents under the supervision of a licensed Occupational Therapist
- Implement individualized treatment plans in accordance with physician orders and facility protocols
- Document patient progress, treatment notes, and outcomes in a timely and accurate manner
- Collaborate with interdisciplinary team members to support resident care goals
- May be required to float to a nearby sister facility if hours are reduced at the primary location
Required Qualifications
- Active COTA licensure in the state of Illinois
- Current NBCOT certification
- Previous experience in a skilled nursing or long-term care setting
- Ability to maintain accurate clinical documentation
Preferred Experience / Skills
- Experience working in a skilled nursing facility environment
- Familiarity with Medicare documentation requirements
- Flexibility to float between facilities as needed
