Key Responsibilities
- Provide occupational therapy treatment services to residents under the supervision of a licensed Occupational Therapist
- Implement individualized treatment plans in a skilled nursing facility setting
- Document patient progress and maintain accurate therapy records in accordance with facility and regulatory standards
- Collaborate with the interdisciplinary care team to support resident goals and outcomes
Required Qualifications
- Current COTA licensure or certification required
- Active state licensure or eligibility to practice in Wisconsin
- Experience working in a skilled nursing or long-term care environment
Preferred Experience / Skills
- Prior SNF or long-term care experience preferred
- Familiarity with Medicare documentation and compliance requirements
