Job Requirements
  • Education Requirements: Valid Montana RN license
  • Certifications Requirements: BLS certification, State RN license
  • Experience Requirements: At least 2 years of care coordination or discharge planning experience; experience with insurance types such as Medicare and Medicaid; referral intake experience
  • Other Requirements: Excellent bedside manner; willingness to commute to Billings, MT; strong communication and organizational skills

Job Responsibilities
  • Provide comprehensive care coordination and discharge planning for patients based on individual needs and insurance guidelines.
  • Assess patient cases to determine appropriate referrals and facilitate referrals intake process efficiently.
  • Maintain accurate documentation of patient interactions, care plans, and insurance information within Cerner and other electronic health record systems.
  • Communicate effectively with multidisciplinary care teams, patients, and insurance providers to ensure seamless care transitions.
  • Participate in ongoing training and team meetings to stay updated on insurance policies, care coordination best practices, and hospital protocols.
