RN Case Manager – Inpatient Acute Care

Must-have requirements

Active California RN license
BLS certification
5+ years of RN Case Management experience
Inpatient acute-care hospital case management experience
Discharge planning experience — must specify the facility/employer where the candidate gained this experience
Schedule: 5×8-hour shifts, every other weekend

Key responsibilities

Inpatient discharge planning and care transitions
Utilization management, level-of-care reviews, and length-of-stay monitoring
Coordinating with physicians, bedside RNs, social workers, and interdisciplinary teams
Preventing avoidable admissions and readmissions
Arranging post-acute care, referrals, and community resources
Documenting assessments, care plans, and discharge arrangements in the EHR
