Job Description:
We are seeking an experienced RN Case Manager for an acute-care hospital in Eureka, CA. The RN Case Manager will be responsible for care coordination, discharge planning, utilization review, patient needs assessment, and arranging post-acute services
Requirements:
2+ years of acute hospital Case Management experience
Recent acute-care Case Management experience within the last year
Experience with Care Coordination & Discharge Planning
Epic experience required
