Prime Staffing is seeking a travel nurse RN Case Management for a travel nursing job in Petaluma, California.

We are seeking an experienced Registered Nurse Case Manager to coordinate care and discharge planning for adult and geriatric patients in an acute-care hospital setting. The RN Case Manager will collaborate with physicians, nursing staff, social services, and other healthcare professionals to support appropriate utilization of hospital services, safe discharge planning, and continuity of care.
This position requires hands-on Epic experience and proficiency with InterQual criteria. The selected candidate will work between the Petaluma and Healdsburg hospital locations, approximately 30 minutes apart, with the weekly assignment split varying according to operational needs.
Key Responsibilities
- Coordinate patient care and collaborate with physicians, nurses, social workers, therapists, pharmacy, and other interdisciplinary team members.
- Develop and coordinate individualized discharge plans based on patient needs and available resources.
- Arrange appropriate post-acute services, referrals, transportation, and follow-up care.
- Perform utilization reviews and assess medical necessity for hospital admission and continued stay.
- Apply InterQual criteria to support level-of-care and utilization decisions.
- Document case management activities, reviews, and discharge plans accurately in Epic.
- Help identify and resolve barriers that may delay discharge.
- Support compliance with CMS regulations, HIPAA guidelines, and facility policies.
- Communicate effectively with patients, families, providers, and community resources to support safe transitions of care.
Required Qualifications
- Active RN license appropriate for the assignment.
- At least 2 years of acute-care hospital Case Management/Discharge Planning experience.
- Previous hands-on Epic experience.
- Experience utilizing InterQual criteria.
- Strong care coordination, utilization review, and discharge planning skills.
- Ability to manage competing priorities and work collaboratively with an interdisciplinary team.
- Availability for day shifts and every other weekend.
- Willingness to work at both Petaluma and Healdsburg locations.
- Travel experience is listed as required; first-time travelers may be considered subject to confirmation.
