Job Description

Seeking an experienced RN Case Manager with acute care hospital experience for a travel assignment supporting two facilities in California.

Required Qualifications

• Minimum 1 year of experience; 2 years listed in the job qualifications.
• Acute care hospital Case Management experience is a HARDSTOP requirement.
• Epic charting experience is a HARDSTOP requirement.
• Experience using InterQual criteria is a HARDSTOP requirement.
• Previous travel experience required.
• Active Compact RN license must be in hand at submission.
• Experience with discharge planning, care coordination, and admission criteria.
• Knowledge of CMS guidelines and HIPAA regulations.
• Experience with adult and geriatric patient populations.

Responsibilities & Skills

• Case management in an acute care hospital setting.
• Care coordination and discharge planning.
• Utilization of InterQual criteria.
• Review of admission criteria and patient care needs.
• Coordination with interdisciplinary teams and support services.
• Collaboration with social services, physical therapy, respiratory services, radiology, pharmacy, and transportation.

Facility & Unit Details

• Facilities: Petaluma Valley Hospital and Healdsburg Hospital.
• Facility Locations:
  • Petaluma: 400 N. McDowell Blvd, Petaluma, CA.
  • Healdsburg: 1375 University Ave, Healdsburg, CA.
• Travel Between Facilities: Approximately 30 minutes apart.
• Patient Population: Adults and geriatrics.
• Daily Census: Approximately 30–35 patients.
• Patient Ratio: Approximately 1:15 on days.
• Weekend Requirement: Every other weekend.
• Floating: Required between both facilities.

The weekly schedule may vary between the two facilities. For example, a traveler may work three days at one facility and two at the other, with the arrangement potentially reversing the following week.

Scheduling & Time-Off Requirements

• All requested time off (RTO) and scheduling requests must be submitted upfront.
• Up to 7 days of RTO may be considered within the stated offer guidelines; additional time off may delay the offer and require client approval.
• Candidates must commit to working 3 of the 5 designated holidays:
  • Thanksgiving
  • Christmas Eve
  • Christmas Day
  • New Year's Eve
  • New Year's Day
• Refusal to float between facilities may result in termination or loss of guaranteed hours.

Submission Requirements

• SSN and DOB required for compliance.
• Compact RN license must be in hand at submission.
• Two professional references covering at least one year of experience within the last three years.
• Candidates must disclose any previous employment or assignments at Providence facilities, including location and dates.
• Only one Providence facility submission is permitted at a time.
• Background screening policy: Facility does not accept Nursys or background-check hits.
• Former staff may return after three months.
