Contract | Days | 5x8 Hours (8:00–16:30) | Variable 5-day workweek, department operates 7 days per week; days off will vary and are not guaranteed to be consecutive
Key Responsibilities
- Perform care coordination and case management for an acute care patient population
- Manage a caseload with a 1:25 ratio in a hospital-based case management setting
- Coordinate care transitions, discharge planning, and utilization review activities
- Float to a sister facility as operationally required; refusal to float may result in termination of assignment
- Commit to working 3 of 5 designated holidays: Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, and New Year's Day
Required Qualifications
- Active, valid California RN license required (pending license accepted, but must be active by start date)
- Minimum of 2 years of acute care Case Management experience performing Care Coordination
- Prior travel nursing experience required — first-time travelers will not be considered
- Must live at least 50 miles from the facility to be eligible as a standard traveler (locals accepted at a different rate)
- RTO requests must not exceed 7 days and must be submitted upfront at time of submittal
- Must provide 2 professional references covering at least 1 year within the last 3 years
Preferred Experience / Skills
- Experience working in a high-volume acute care hospital environment
- Familiarity with utilization management and payer requirements
- Strong communication and interdisciplinary collaboration skills
