Join a leading healthcare organization as a Case Manager Registered Nurse in Petaluma, CA. This contract position offers an exciting opportunity for RNs with experience in case management, particularly those familiar with EPIC systems.

As a Case Manager RN, you will float between Petaluma and Healdsburg, which are approximately 30 minutes apart. Your schedule may vary, with potential assignments of 3 days at one location and 2 days at another, or a full week at a single site depending on staffing needs and PTO. This role includes every other weekend.

Key Responsibilities:

• Utilize EPIC for patient management and documentation.
• Coordinate care plans and communicate effectively with patients and healthcare teams.
• Float between facilities as required, adapting to different environments and teams.
• Ensure compliance with healthcare regulations and standards.

Requirements:

• Registered Nurse (RN) license in California.
• Experience in case management.
• EPIC experience is a must.
