Job Opportunity – Registered Nurse, Case Manager – Eureka, CA 95501

Start Date: October 19, 2026

Location: Eureka, California – Northern California coastal region

Schedule: Day shift, 5x8-hour shifts (08:00 - 16:30), every other weekend required. Caseload up to 25 patients. No call required. May float between units based on departmental needs.

Requirements:

• Active California RN license or Compact license (must be in hand at submission)
• Minimum 2 years of acute care hospital case management experience required
• Current acute care case management experience within the last year (required)
• Epic charting system experience required
• Experience with HMOs, IPAs, advance directives, POLST, and protective services processes
• Knowledge of admission criteria, discharge planning, DRG, appeals and denials, and care coordination
• Familiarity with CMS guidelines, HIPAA regulations, and Joint Commission standards
• Two professional references covering one year within the last three years
• Willingness to work 3 of 5 major holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day)
