Start Date: October 5, 2026

Location: Mission Viejo, California

Schedule: Day shift, 5x8-hour shifts (08:00 - 16:30). Variable 5-day workweek with department operations occurring 7 days per week. Days off will vary and are not guaranteed to be back-to-back, although every effort will be made to schedule consecutive days off when possible. Must commit to working 3 of 5 major holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day).

• 91-day travel assignment with 40 hours of orientation
• Focus on care coordination and discharge planning, not utilization review
• Floating expectation to nearby sister facilities as needed
• Work with diverse patient populations including adults and geriatrics
• Dress code: Turquoise scrubs or business casual attire

Requirements:

• Active California RN license (pending license accepted but must be active by start date)
• Minimum 2 years of acute medical Care Manager experience performing care coordination
• Previous travel nursing experience required (first-time travelers will not be considered)
• Epic charting system experience is mandatory
• Experience with patient assessment, care coordination, discharge planning, and family meetings
• Knowledge of HMOs, IPAs, adoption/foster care, substance abuse treatment, homelessness resources, mental health crisis management, advance directives, and protective services
• Competent with arranging DME, SNF placement, and ambulance transport independently
• Trauma Level I or II experience preferred
• Two professional references covering 1 year within the last 3 years
• Clean background check required
