Job Opportunity – Registered Nurse, Case Manager – Mission Viejo, CA 92691
Start Date: October 5, 2026
Location: Mission Viejo, California
Schedule: Day shift, 5x8-hour shifts (08:00 - 16:30). Variable 5-day workweek with department operations occurring 7 days per week. Days off will vary and are not guaranteed to be back-to-back, although every effort will be made to schedule consecutive days off when possible. Patient ratio 1:25. 40 hours of orientation provided.
Highlights:
- 13-week travel assignment at an acute care facility serving adult and geriatric populations
- Focus on care coordination and discharge planning – no utilization review responsibilities
- Opportunity to work with Epic charting system in a fast-paced acute care environment
- Floating may be required to sister facilities in the region
- Must commit to working 3 of the 5 major holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day)
- Professional dress code: turquoise scrubs or business casual attire
Requirements:
- Active California RN license (pending licenses accepted but must be active by start date)
- Minimum 2 years of acute medical Care Manager experience performing care coordination
- Previous travel nursing experience required – first-time travelers will not be considered
- Epic charting system experience required (hardstop requirement)
- Experience with patient assessment, care coordination, discharge planning, and family meetings
- Knowledge of HMOs, IPAs, adoption/foster care, substance abuse treatment, homelessness resources, mental health crisis management, advance directives, POLST, and protective services
- Competent with arranging DME, SNF placement, and ambulance transport independently
- Two professional references covering 1 year within the last 3 years required
- Clean background check required (no exceptions)
- Time off requests limited to no more than 7 days and must be submitted upfront during application
