Registered Nurse Case Manager

Schedule: Days | 5×8-hour shifts | 40 Hours/Week
Weekend Requirement: Every Other Weekend (EOW)

Travel Experience: Required; first-time travelers may be considered with Clinical/Manager approval
Required Experience: 2 years of acute hospital Case Management experience

Position Overview

The Registered Nurse Case Manager will provide comprehensive case management services in an acute hospital setting, focusing on care coordination, utilization review, discharge planning, and ensuring patients receive appropriate resources throughout their hospital stay.

Required Qualifications

  • Minimum 2 years of acute hospital Case Management experience.

  • Acute-care hospital Case Management experience must be current within the last year.

  • Experience with care coordination and discharge planning.

  • Experience working with HMOs, IPAs, and other managed-care organizations.

  • Experience with advance directives, POLST, and protective services processes.

  • Must be proficient with Epic.

  • Travel experience required; first-time travelers may be submitted for Clinical/Manager review if they are a strong candidate.

  • Must be willing to work every other weekend.

  • Must be willing to float between like units as needed.

Key Responsibilities

  • Perform comprehensive case management and patient needs assessments in an acute hospital setting.

  • Coordinate patient care across multidisciplinary teams, including physicians, nursing, social work, and other healthcare professionals.

  • Develop and coordinate safe and appropriate discharge plans based on patient needs and available resources.

  • Conduct utilization review and evaluate patients against admission and continued-stay criteria.

  • Coordinate DME, home health, post-acute care, rehabilitation, skilled nursing, and other discharge services.

  • Assist with benefits eligibility, prior authorizations, appeals, and denials as applicable.

  • Facilitate communication with insurance companies, HMOs, IPAs, providers, patients, and families.

  • Ensure compliance with CMS, HIPAA, The Joint Commission, Core Measures, and National Patient Safety Goals.

  • Assist with advance directives, POLST, and protective services processes when required.

  • Document case management activities accurately in Epic.

Acute Hospital Specialty Experience

Experience with case management involving the following patient populations/settings is beneficial:

  • Emergency Department

  • ICU

  • Medical-Surgical

  • Pediatrics

  • SDU / PCU / IMC / Observation

Patient Population: Adolescents, Adults, Pediatrics, Geriatrics

Critical Requirements – HARD STOP

Candidates must have:

  • 2 years of acute-care hospital Case Management experience.

  • Acute hospital Case Management experience within the last year.

  • Experience with care coordination and discharge planning.

  • Experience with HMOs, IPAs, or similar managed-care organizations.

  • Experience with advance directives, POLST, and protective services processes.

  • Epic experience.

  • Willingness to work every other weekend.

Do not submit candidates who are missing the required experience or skills. Strong candidates may be screened for Clinical/Manager approval.

RTO / Cancellation Policy

  • Please include all requested time off (RTO) at the time of submission.

  • A winter bill-rate increase will be effective from October 4, 2026 through February 27, 2027.

  • After February 27, 2027, the bill rate will automatically return to the standard rate.

  • Cancellations related to the rate change after contract signature will result in a systemwide DNR (Do Not Return).

Parking

  • Free parking is provided on the first day.

  • Assignment parking information will be provided during orientation.

Housing

The facility has multiple housing resources available for travelers. If a traveler has difficulty securing housing, the facility may assist with the search and can potentially connect the traveler with a realtor.
