Job Opportunity – Registered Nurse, Case Manager – Apple Valley, CA 92307

Start Date: November 2, 2026

Location: Apple Valley, California

Schedule: Day shift, 5x8-hour shifts (08:00 - 16:30), every other weekend rotation required. Patient ratios: 1:25 weekdays, 1:45 weekends. 40-hour orientation provided.

Highlights:

• Acute care case management and discharge planning role in a well-established facility
• Collaborative interdisciplinary team including Physical Therapy and Social Services
• Work with diverse patient populations including neonates, adolescents, adults, and geriatrics
• Epic charting system utilized for documentation
• Business attire or scrubs accepted
• No floating required within the unit

Requirements:

• Active California RN license (pending licenses accepted if active by start date)
• Minimum 2 years of acute care case management or discharge planning experience required
• Hospital experience required; behavioral health experience does not substitute
• Previous travel nursing experience required; first-time travelers cannot be considered
• Experience with patient assessment, care coordination, discharge planning, and utilization review
• Knowledge of HMOs, IPAs, and CMS guidelines
• Competency in arranging DME, SNF placement, and ambulance transport independently
• Familiarity with adoption/foster care, substance abuse treatment, homelessness resources, mental health crisis intervention, advance directives, and protective services processes
• Two professional references covering one year within the last three years
• Clean background check required
