Job Opportunity – Registered Nurse, Case Management – Apple Valley, CA 92307

Start Date: October 19, 2026

Location: Apple Valley, California

Schedule: Day shift, 5x8-hour shifts (08:00 - 16:30). Weekend rotation required (every other weekend). Patient ratios: 1:25 on weekdays, 1:45 on weekends. No floating required.

Requirements:

• Active California RN license required at start (pending licenses accepted during application process)
• Minimum 2 years of acute care case management or discharge planning experience in a hospital setting
• Previous travel nursing experience required; seasoned travelers preferred
• Hospital-based case management experience mandatory; behavioral health experience does not substitute
• Experience with patient assessment, care coordination, discharge planning, and utilization review
• Competency in arranging DME, SNF placement, and ambulance transport independently
• Knowledge of HMOs, IPAs, adoption/foster care, substance abuse treatment, homelessness resources, mental health crisis management, advance directives, and protective services
• Ability to organize and participate in family meetings and treatment plan development
• Proficiency with Epic charting and computerized documentation required
• Two professional references covering one year of experience within the last three years
