Job Opportunity – Registered Nurse, Case Management – Apple Valley, CA 92307
Start Date: October 12, 2026
Location: Apple Valley, California
Schedule: Day shift, 5x8-hour shifts (08:00 - 16:30). Weekend rotation required (every other weekend). Patient ratios: 1:25 on weekdays, 1:45 on weekends. No floating required.
Highlights:
- 13-week travel assignment at an acute care facility with a 40-hour orientation period
- Work with interdisciplinary support including Physical Therapy and Social Services
- Serve diverse patient populations including neonates, adolescents, adults, and geriatrics
- Business attire or scrubs acceptable; professional case management environment
- Epic charting system utilized for computerized documentation
- Winter rate increase effective through February 27, 2027
Requirements:
- Active California RN license (pending license accepted but must be active by start date)
- Minimum 2 years of acute care case management or discharge planning experience required
- Hospital experience mandatory; behavioral health experience does not substitute
- Seasoned travel nurse experience required; first-time travelers cannot be considered
- Experience with patient assessment, care coordination, discharge planning, and utilization review
- Proficiency with HMOs, IPAs, and arranging DME, SNF, and ambulance transport independently
- Knowledge of foster care, substance abuse treatment, homelessness resources, mental health crisis intervention, advance directives, and protective services
- Ability to organize and participate in family meetings and treatment plan development
- Two professional references covering one year within the last three years
- Must commit to working 3 of 5 major holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day)
- Clean background check required; no exceptions for background discrepancies
