Description

American Traveler is hiring an experienced RN Case Manager for an acute-care hospital assignment requiring Epic experience and a rotating weekend schedule.

Details

  • Acute-care hospital setting within the Case Management department
  • Manages a caseload of hospitalized patients with a 1:25 patient ratio
  • Involves care coordination, discharge planning, needs assessment, DME ordering, and prior authorizations
  • Requires knowledge of appeals and denials, benefits eligibility, and utilization review processes
  • Applies CMS and HIPAA regulatory guidelines
  • Serves acute care specialties including ICU, Med-Surg, and SDU/PCU/IMC/Observation units
  • Uses Epic EMR for documentation
  • Schedule consists of four 10-hour day shifts with a rotating weekend requirement
  • Floating required to other areas, including other facilities within a 75-mile radius

Requirements

  • Active RN license (Standard or Compact) required
  • BLS certification required; CCM certification preferred
  • Minimum of 1 year of acute-care hospital Case Management experience required
  • Epic charting system experience required
  • Experience with MCG or InterQual guidelines preferred
  • Strong clinical assessment, care coordination, discharge planning, and communication skills required
  • Basic to intermediate computer skills required
  • Current driver's license or state-issued ID and a utility bill with matching address dated within the last 3 months required for consideration
  • Two supervisory references required for consideration
  • Any gaps in work history of 3 months or more must include an explanation

Additional Information

  • Coordinate care for complex hospitalized patients and facilitate timely transitions of care
  • Work independently while collaborating within a multidisciplinary care team
  • Document accurately and thoroughly in the electronic medical record
  • Local candidates are not eligible; permanent address must be located more than 75 miles from the facility
  • Facility intends to consider converting candidates to permanent staff upon completion of the assignment
  • Former INTEGRIS employees must have a minimum of 1 year separation from the health system to be eligible
  • Maximum of 5 days RTO allowed per 13-week assignment, with no holiday RTO permitted
  • No block scheduling requests will be accommodated
  • First-time travelers are not considered for this position
