Description

American Traveler is hiring a Case Management RN with BSN, CCM, and acute care experience for an ortho unit assignment requiring an active MA RN license.

Details

• Case Manager role on an ortho unit within an acute care hospital
• Day shift schedule, 8:00 AM to 4:30 PM, 5x8-hour shifts
• Every 3rd weekend required
• Union facility with no special scheduling accommodations - must be able to work all 7 days
• Uses Epic for charting

Requirements

• Active MA RN license required
• 3-5 years of RN experience required
• BSN required
• BLS certification required
• CCM certification required for consideration
• Acute care experience required
• Epic charting experience preferred

Additional Information

• Responsibilities include care coordination, discharge planning, needs assessment, and DME ordering
• Familiarity with CMS and HIPAA regulatory guidelines expected
• Must complete a Pass/Fail Tele test during orientation with an 80% passing score and only 1 attempt allowed
• Must also complete a Pass/Fail Medication exam at orientation
• Additional compliance modules required pre-start, during orientation, and annually
• Local candidates accepted within a 75-mile radius at the same bill rate
• Returning staff must have a 12-month separation from the health system
• Up to 14 days of RTO may be submitted for review, with no RTO allowed during the first week
