Key Responsibilities:

• Provide comprehensive case management services to patients.
• Coordinate care across multiple disciplines and settings.

• Float to sister campuses as needed.

Requirements:

• 2+ years of Case Management experience required.
• Current RN License required; nursys must be on profile.
• BLS certification required.
• Care Management certification (AMC or CCM) preferred.
• Degree accreditation must be on profile.
• Willingness to float to sister campuses as needed.
• All RTO must be provided at submission; no approvals after submission.
