• Floating: Must be willing to float to other units and hospitals as needed

Job Requirements

• Active Registered Nurse license in Alabama or a compact license required; temporary licenses are not accepted
• Minimum 2 years of nursing experience required
• Experience in utilization management and/or case management preferred
• Associate degree in Nursing required; BSN or related bachelor's degree in a healthcare field preferred
• Current AHA BLS or American Red Cross certification required
• Strong communication, organization, and teamwork skills
• Ability to work collaboratively with interdisciplinary healthcare teams
• Willingness to float to multiple units and hospitals

Responsibilities

• Coordinate patient care and services across the acute care episode and along the continuum
• Collaborate with internal and external healthcare team members to support effective patient care
• Help manage utilization of healthcare resources to support efficient, high-quality outcomes
• Assist with safe discharge planning and transition planning for patients
• Monitor care needs and communicate updates to support timely clinical decisions
• Promote positive clinical and financial outcomes through proactive care coordination
• Support patient-centered care in a fast-paced acute care environment
