Maintains an active patient registry. Monitors psychiatric and substance misuse symptoms and other measures to track outcomes. Actively facilitates referrals to address behavioral health and social determinant needs. Screening to assess an individual's potential need for services. Crisis intervention and stabilization for individuals in acute crisis. Evaluation based on the American Society of Addiction Medicine Criteria. Development of a clinical service plan in collaboration with the client based on evaluation. Facilitates referrals to substance use disorder treatment and recovery support. Ongoing assessment and support in collaboration with external service providers. Ongoing follow-up and support for clients engaged in services. Completion of interviews and documentation of client-level data at multiple intervals. Preference is given to a degree that is certifiable or licensable. No weekends or on-call requirements. On-site work only; remote work is not an option.
