Join a leading acute rehabilitation hospital as a Case Manager for our adult acute care unit. This facility is dedicated to the treatment and recovery of individuals who have experienced severe injuries or illnesses. We are seeking a seasoned Case Manager with experience in acute hospital settings to provide exceptional care and support to our patients.

Position Information:

• Role: Case Manager (RN, SW, PT, OT, ST, RT with CM experience)
• Required Licensure: Current Registered Nurse or Social Work licensure or Healthcare professional licensure as a Respiratory Therapist, Physical Therapist, Speech Language Pathologist, or Occupational Therapist with appropriate Case Management experience.
• Weekly Guaranteed Hours: 40
• Weekend Hours: 0
• Call Requirements: 0
• Required Certifications: Case Management experience is required; CM certification is preferred (e.g., CCM, ARN, ACM, or ANCC RN-BC).
• Years of Experience: 2-3 years preferred, open to discussion based on individual experience.
• Float Requirements: 0

Unit Information:

• Number of Beds: 60
• Patient Ratios: 1:15 (+/-)
• EHR: UDS PRO Medi Tech and Pinpoint
• Patient Population: Primarily adult patients with acute rehabilitation diagnoses, including stroke and other medical debilitation.

Must Have Skills/Experience:

• Case Management experience
• Effective communication skills
• Utilization review and ability to support team and family conferences for acute rehabilitation

Preferred Skillset/Experience:

• Basic computer skills (Excel, Word, Outlook)
• Strong organizational and time management skills
• Ability to work collaboratively with nursing, therapy, and medical teams

Personality Fit: Communicative and able to work well with a diverse team of professionals. Must understand how to balance patient care advocacy with resource stewardship and maintain professional communication in challenging discussions with families and patients regarding healthcare realities.
