Description

American Traveler is hiring a Case Manager RN for an inpatient care management role requiring BSN and prior care management experience.

Details

  • Inpatient hospital setting within the Care Management department
  • Oversee a daily caseload of inpatient patients with varied care needs
  • Attend interdisciplinary rounds and complete initial patient assessments
  • Collaborate with patients, families, caregivers, and the healthcare team to develop plans of care
  • Coordinate post-discharge needs such as home health and inpatient rehab with post-acute referral sources
  • Document and place orders for home health, DME, and nutritional needs within Epic EMR
  • Day shift schedule, Monday through Friday, 8:00 AM to 4:40 PM
  • No weekend or on-call requirements

Requirements

  • Active RN license (NH or Compact) required
  • BSN required
  • Certified Case Manager (CCM) certification required
  • Minimum 3 years of inpatient clinical experience
  • At least 1 year of Care Management experience
  • Recent Level 1 or comparable Level 2 Trauma experience within the last 2 years
  • Epic EMR experience preferred
  • BLS certification required

Additional Information

  • Candidates must reside outside a 75-mile radius of the facility to be considered
