Parkview Physicians Group is currently seeking experienced Board Eligible / Board CertifiedFamily Medicine or Internal Medicine physicians to join our Population Health team in Fort Wayne, Indiana.

Specifics of The Role

  • Full time and part time opportunities available
  • Hours are Monday-Friday from8am - 5pm
  • Involves travel by conducting daily rounding on Parkview patients in high performing network skilled nursing facilities
  • Nurse practitioners cover call; physician would need to be available as a backup by phone

The Team

  • Group consists of 1 physician & 5 advanced practice providers that are dedicated to providing quality continuation of care to patients that are discharged from the hospital
  • Comprehensive care team and clinical support model comprised of medical assistants and registered nurses
