    Transition Planning Manager

    Location: Bronx, NY 10463 (Riverdale area)
    Employment Type: Full-Time | Permanent
    Schedule: Monday – Friday, 9:00 AM – 5:00 PM (rotating holidays)
    Compensation: $67,000 – $72,000 per year + Full Benefits

This organization focuses on delivering high-quality, patient-centered care and ensuring safe, coordinated transitions from facility to home and community-based settings.

    Position Summary
    The Transition Planning Manager plays a critical role in discharge planning and care coordination for a geriatric population. This individual ensures patients transition safely back into the community with the appropriate services, resources, and support systems in place.

    Key Responsibilities

    • Coordinate and arrange home care services and community-based supports for patients

    • Collaborate with interdisciplinary teams to ensure safe and effective discharge planning

    • Provide emotional and social support to patients and families throughout the transition process

    • Educate patients and families on benefits, entitlements, and available resources

    • Assist with benefit applications, renewals, and ongoing eligibility maintenance

    • Maintain accurate, timely, and compliant documentation of all transition planning activities

    • Ensure continuity of care and follow-up services post-discharge

    Qualifications

    • Master’s degree in Social Work (MSW) required

    • Minimum of 2 years of experience in geriatrics, discharge planning, or case management preferred

    • Experience working with geriatric populations and providing high-level customer service

    • Strong organizational, documentation, and time management skills

    • Excellent communication skills with the ability to work collaboratively across care teams

    • Ability to manage multiple cases and prioritize effectively in a fast-paced environment
