    JOB SUMMARY:

    The Transformation of Rural Community Health (ToRCH) Pilot Coord leads, tracks, and reports on all phases of the ToRCH program’s strategic priorities, including addressing social determinants of health (SDoH) to improve community health,  developing and maintaining robust community partnerships, and serving on the  program’s leadership board to meet the program’s incentivized goals of improving the  healthcare experience and reducing rates of avoidable hospitalizations and readmissions.

    The ToRCH Pilot Coord reports directly to the Director of Population Health.


    QUALIFICATIONS:

    Education:
    • Bachelor's degree in Nursing, Public Health, Healthcare Administration, or experience in a related field preferred.   

    Licensure/Certification:
    • None

    Work Experience:
    • Experience specific to Health Equity topics in healthcare.
    • Demonstrated competency in a wide variety of Health Equity topic areas. 
    • Demonstrated ability to make diversity, equity, and inclusiveness concepts assessable and relevant.
    • Demonstrated ability to work with people from all walks of life.
    • Have strong communication and relationship building skills with all levels of employees and management. 
    • Must be organized and have engaging presentation skills in both virtual and in-person settings.
    • Comfortable with the use of technology.  

    RESPONSIBILITIES/JOB DUTIES: 

    Contribute to an organizational culture of respect, hope, and personal accountability regarding ToRCH.

    Oversees and maintains ToRCH program initiatives aimed at integrating resources and social care supports within rural communities to address the upstream causes of poor health and improve health at the population level.

    Allocates and delegates resources to efficiently manage day-to-day program operations, effectively support initiatives, and achieve program milestones. 

    Monitors and assesses progress toward program objectives, conducts regular program review, creates status reports for the program leadership team, analyzes data, and troubleshoots problem areas. 

    Documents and monitors timelines and deliverables, works to resolve risks, and mitigates barriers. Develops and delivers status reports, requirements documentation and presentations in both formal and informal settings to the program’s leadership board, senior leadership and other key stakeholders. 

    Serves as non-voting member of the ToRCH Leadership Board. Efficiently plans and facilitates meetings of key stakeholders, using standard meeting management tools and techniques.

    Compiles documentation for submission to accrediting/regulatory agencies as defined by the project.

    WORKING CONDITIONS:

    Direct patient, employee, volunteer, and community contact with possible exposure to infections, communicable diseases and blood borne pathogens.

    Occasionally subjected to irregular hours.

    Subjected to frequent interruptions.

    Works indoors in well-lighted and ventilated environment.
