What Youll Do:
- Provide psychosocial assessment and support to hospital patients
- Develop and implement care plans tailored to patient needs
- Collaborate with healthcare teams to enhance patient outcomes
- Support discharge planning and coordinate community resources
- Document patient interactions accurately in compliance with hospital standards
- Participate in team meetings and case reviews
- Contribute to a small, close-knit unit with a focus on quality care
What You Need:
- Active Massachusetts LMSW license
- Demonstrated hospital social work experience
- Strong interpersonal and communication skills
- Ability to work independently and in a team environment
- Dependability and professionalism
