Employee Separation Policy: Any past employee of UPH whether FTE, or PRN for the UPH float pool, cannot work travel in another Market no matter which facility they currently work at, or have worked at within the past 12 months. If they worked travel at UPH (not FTE or PRN), the separation policy does not apply
HOLIDAY update: unless the Unit is closed, your contractor is required to work 1-2 of 3 upcoming Winter holidays
- NBO: 16 hrs
- EMR: EPIC
- OT/Holiday: 1.15x multiplier
- Call Back: 1.5x multiplier with 2 hr CB minimum
- On Call: ***/hr
- Charge/Precept: ***/hr
Billable OT:
- 36-40 hrs/week, OT billable after 40
- 48 hrs/week, OT billable after 48

- Equipment: Airvo 2, Airvo 3, V60, Nkv-330, Hamilton T-1, Drager V500, Drager V800, Vapotherm, Bubble CPAP (homemade units) and ABL 90 for ABGs
- Patient Types: Adult ICU & NICU
- Bed Types: 12 ICU
- Scrubs: any color tops and bottoms, solids
- CRTs accepted: ***/hr bill rate

- 15 months of Respiratory experience in Hospital setting [required]
- recent experience with NICU patients [required]

- IA/Iowa Respiratory license {highly preferred} -- will accept licensing candidates; 1-3 week turnaround time

PROFILE REQUIREMENTS:
- Work history/resume/employment history must have 36 months of current RRT / Respiratory Therapy experience [required]
- Skills Checklist must be specific to the discipline listed on the job (Respiratory Therapy) [required]
- IDPH / Iowa Department of Public Health / Respiratory Care and Polysomnography [highly preferred] [document]
- Professional Reference/Reference Check - charge/supervisor/nurse manager/director/CNO/COO performance evaluation/reference from within 24 months of today's date [preferred]
- Driver s License [document] [required]
- AHA/American Heart Association or ARC/client BLS / basic life support [required]
- AHA/American Heart Association advance cardiac life support [required]
