CERTIFICATION REQUIREMENTS :
STATE LICENSE REQUIREMENTS :
ADDITIONAL LICENSE REQUIREMENTS : must be willing to work/take call through the Christmas holidays.
Weekend Requirements : call
On Call Requirements : 1 weekend/month & 3-4 nights/week

Cases per day vary due to service line: 1-4 or 5-12 Endos
NO LOCALS. Travelers must list 50+ miles from facility. Please include perm address in profile.

- 2 years current OR experience [REQUIRED]
- Must have CIRCULATING experience [REQUIRED]
- Scrub experience is a plus, but not required [HIGHLY PREFERRED]
- Must be proficient in the following cases: Ortho, General, Endo, Sports Medicine, Gyn, C-Sections
- Total Joints experience [REQUIRED]
- Previous travel experience [HIGHLY PREFERRED]
- Work History
- Operating Room skills checklist [REQUIRED][DOCUMENT]
- 1 Supervisory Signed Reference, from the past year of employment [REQUIRED]
- Active American Heart Association BLS - [REQUIRED][DOCUMENT]
- Active American Heart Association ACLS - [REQUIRED][DOCUMENT]
- Active American Heart Association PALS - [REQUIRED][DOCUMENT]
- License verification [REQUIRED][DOCUMENT]
- All licenses/certs must be current at time of submission
- Permanent address must be included in submission file [REQUIRED]
- RTO - please be mindful and limit RTO requests as this is a small, critical access facility.
