Accurately and legibly documents all patient care activities, including but not limited to, wound assessment, patient care teaching, demonstrated progress toward goals, medication administration, nursing flow sheets and all assessment forms. Utilizes assessment data to identify patient care needs, to include patient education and discharge planning.

Travelers may not take all holidays off. MUST work TWO blocks (if scheduled/needed):

Thanksgiving Day/Friday after and weekend after
Christmas Eve through Christmas Day and weekend after
New Year Eve through New Year Day and weekend after

"Local" travelers - who reside within 50 miles of facility, will not be considered for this need

• Ability to formulate an individual plan of care, revise plan as indicated by the patient's response to treatment and evaluate overall plan for effectiveness.
• Coordinates and supervises patient care, as necessary.
• Demonstrates an ability to organize and prioritize job function.
• Utilizes assessment data to identify patient care needs, to include patient education and discharge planning.
• Recognizes and reports abnormal assessment data and incorporates findings into the plan of care: validates and revises expected outcomes based on assessment.
• Accurately and legibly documents all patient care activities, including but not limited to, wound assessment, patient care teaching, demonstrated progress toward goals, medication administration, nursing flow sheets and all assessment forms.
• Delegates tasks to the health care team members appropriately.

ADDITIONAL LICENSE REQUIREMENTS : Active NH or Compact License is required at time of submission. Facility will not pend RN license.
Weekend Requirements : Every other weekend/Holidays required if needed
